Case Manager Position Description

Essential Functions

Provide direct case management services to client’s that includes:

  • Initial screening and interviewing placement process.
  • Guidance in planning for future goals and setting attainable objectives.
  • Providing support, encouragement, and accountability to clients in their daily activities toward reaching the goal of self-sufficiency.
  • Conducting random client drug tests.
  • Providing substantial client direction regarding essential life skills such as budgeting and the monitoring of client’s savings accounts.
  • Client referrals to community resources.
  • Functioning as a liaison between client and external agencies.
  • Management and maintenance of client progress.
  • Providing conflict management, crisis intervention, and enforcement of program rules and regulations with each client as needed or required.
  • Supervising House Manager regarding enforcement of house rules; assignment of chore duties, laundry schedule, visitors policy, zero tolerance drug/alcohol policy, general cleanliness of units and exterior premises.
  • Conduct client exits of guests according to established policies of Colette’s Children’s Home.

 

Additional Responsibilities

  • Produce reports as specified by governmental funding requirements or requested by Colette’s Children’s Home management.
  • Attend training classes or meetings as required or as needed to maintain overall program integrity, individual client success, and fulfill agency requirements.
  • Maintain communication with CCH Program Supervisor and other staff members by attending weekly staff meetings and completing written reports when necessary.

Job Title: Fund Development Assistant

Job Summary:

CCH is searching for a passionate individual with great interpersonal skills to go out into the community and find, cultivate, and maintain donor relationships. A fundamental part of the position will be to retain new donors, increase their involvement, and raise total contributions. Must be comfortable with public speaking and available to work nights and weekends. Under the supervision of the Executive Director and current Fund Development staff, the Fund Development Associate will also help ease the workload of the current Fund Development staff by writing grants, maintaining funder data, and performing miscellaneous tasks as assigned.

Hours:

40 hrs. week. Must be flexible and available nights and weekends to attend events, meetings, etc. upon request, sometimes with late notice.

Job Responsibilities:

*Finding, creating, and maintaining donor relationships and partnerships
*Attend funder related meetings and represent the agency in a variety of community settings, including funder, provider, and community meetings or events. Many times, on nights and weekends.
*Work independently and with the Executive Director to research prospective funders and develop donor relationships.
*Meet face-to-face with potential donors and cultivate major gift prospects.
*Give professional presentations representing Colette’s Children’s Home when groups request a speaker.

Grant Writing:

*Research new grant opportunities
*Write grant proposals for foundation, corporation, and government agency grants.

Support current Fund Development Staff:

*Writing and sending out thank you/ receipt letters
*Keeping donor information up to date in GiftWorks
*Data entry
*Help with the design and preparation of newsletters.
*Coordinating donation drives
*Perform other miscellaneous duties as assigned

Qualifications and skills:

*Ability to effectively and professionally represent the agency and our mission
*Experience public speaking to large groups
*Must be outgoing
*Experience developing relationships and soliciting donations from funders
*An understanding of the individual relationships required to secure major and corporate gifts
*Exceptional written and verbal communication skills
*Computer and internet competency (Excel, word, etc.)
*Valid Driver’s License, reliable transportation, ability to pass a background check
